An androgen hormone that women have in small amounts, often higher or more active in PCOS.

Why it matters for PCOS

Testosterone is the main androgen women produce in small amounts. In PCOS, higher testosterone levels or higher androgen activity are common, and they can contribute to acne, hirsutism, oily skin, and scalp hair thinning. Excess androgen is one of the core features in the diagnostic criteria for PCOS.

A blood test usually reports total testosterone and free testosterone. Total testosterone is the whole amount in the blood, while free testosterone is the part not bound to SHBG and active on tissues. These results are read together with your symptoms and cycle pattern, not on their own, so ask a doctor what they mean for you.
